Chess at PS 6 Program Registration (Fall 2026)

Fall 2026 Semester. Open to PS 6, Grades K-5 students. A tuition invoice will be generated automatically once you submit your enrollment request form. To confirm and reserve your enrollment in the program – payment is due upon receipt of invoice. Enrollment is complete when your payment has been processed. You’ll receive an email Enrollment Confirmation once your payment has been processed. Please allow up to 24 hours for processing. Thank you for your interest in the chess program at PS 6. It is the responsibility of the Parent/Guardian to inform Chess & Education, in writing, if someone cannot pick up your child. If someone other than the listed persons will be picking your child up, you must notify Chess & Education by phone call and in writing (text message or email). You must provide Full Name, a description of the authorized person, their relationship to child and a short note from parent/guardian only, giving this authorization. Please do this in advance. We will not release child to anyone not on your authorized list or without a phone call and written authorization as indicated here. If registration is canceled prior to the start of the semester, a full refund of the amount paid will be provided. If a student would like to withdraw from the program after the 1st or 2nd class, we will prorate the class/es and process a refund for the remaining. We do not give refunds after the second class. I understand that I am not be entitled to any refunds or deductions for any absences during the term. Classes are not transferable. Chess & Education will not give refunds, nor offer make up days or credit for class absences or any class/es that have been cancelled due to any event that is out of Chess & Education’s control.   • AFTER SCHOOL CODE OF CONDUCT From: After School Coordination Our Goal: Our After School program offers a wide range of extracurricular activities designed to support the diverse interests of our students and foster their development through engaging classes. This program is an emotionally safe environment for kids to grow! We prioritize the safety and well-being of your children and the other children participating in the program. This Code of Conduct, which governs the After School programs that take place at PS6, is separate from the New York City Public Schools Citywide Behavioral Expectations to Support Student Learning Grades K–5 including the K–12 Student Bill of Rights and Responsibilities and the Discipline Code, and this Code of Conduct governs participation in After School programming offered at PS6, which is not managed by the New York City Department of Education. Families participating in After School programming should review this Code of Conduct with their participating students. If you have questions about any terms, please reach out to afterschoolcoordinator@ps6pta.com before signing and acknowledging acceptance of these terms. Students may not participate in After School programming unless families have accepted the Code of Conduct terms. PART A: CODE OF CONDUCT Safety, Behavior and Participation All students, parents, guardians, and caregivers are expected to adhere to the following standards. 1. Respect for others – Students, parents, guardians, and caregivers will treat fellow students, instructors, and staff with kindness and respect, and use appropriate and courteous language at all times. 2. No disparaging remarks – Disparaging comments or remarks, including those related to race, social status, nationality, and/or orientation, are not permitted. 3. Peaceful conflict resolution – Students are expected to resolve conflicts by engaging with staff members and asking for their help, guidance, and support. 4. Physical boundaries – Public exposure of self, unwanted or aggressive physical contact is strictly prohibited, including grabbing, hitting, slapping, or pushing. Students who experience unwanted physical contact should report it to a staff member immediately. 5. Following directions – Students will behave in a manner conducive to learning and participation and will not disrupt class. 6. Receptiveness to feedback – Students, parents, guardians, and caregivers are expected to receive and respond constructively to feedback regarding behavior, safety, or health concerns. After School Progressive Discipline Model After School follows a 5 step progressive discipline model to guide students in making positive choices and acting in a safe and responsible manner. When behavioral challenges arise, After School program providers/teachers and coordinators are expected to take appropriate disciplinary action and communicate with parents about concerns. This model applies to all students, including those with disabilities. Disciplinary responses may involve: 1. Teacher redirection and a conversation with the student. 2. Support from After School Coordination staff, and a walk to release energy and reflect. 3. Communication with the child’s parent, guardian, or caregiver, and early pickup if necessary. 4. Formal family meetings following repeated incidents and/or ongoing concerns. If necessary, a temporary suspension may be issued. In such cases, the student will be suspended from the specific class in which the issue occurred. Any suspension will take effect starting with the next scheduled class for the After School program. 5. Removal from the program, if all prior interventions are unsuccessful. The progressive discipline model is intended to be applied for routine behavioral issues. Incidents that escalate or pose a risk to the physical or emotional well-being of students or staff may result in immediate suspension or dismissal from the program or skip steps in the progressive disciplinary model. Students may be immediately suspended while the length of the suspension is determined by the After School Coordination staff, the After School provider/teacher, and PS 6 administration through a full review. The length of the suspension is at the discretion of After School Coordination staff, the After School provider/teacher, and PS6 administration after a full review of the incident. Suspensions may range from short-term (removal from 1-2 class sessions) to longer- term (removal from 4-5 class sessions). Depending on the nature of the offending conduct, a child may be suspended from the After School program during which the conduct took place or all After School programming. This decision will follow the review process described above. Students who are suspended from the After School program must be picked up from school at normal school dismissal time (2:30pm). Neither PS6 nor the After School program will be responsible for providing substitute childcare for students who are suspended from the After School program. There is no refund for missed days due to a suspension. If a student is dismissed from a class permanently, a pro-rated refund will be provided. I have read and agree to the Code of Conduct in Part A above. I have explained the Code of Conduct to my Child.
  • PART B: DISMISSAL PROCEDURES • Dismissal starts at 5:00pm and ends strictly at 5:45pm. Parents, guardians, and/or caregivers should make a consistent and conscious effort to pick up between these times. Excessive and/or consistently late pick-up after 5:45pm may lead to a child’s removal from the After School program. parents please reach out to the afterschool coordination phone if there is a delay and you cannot pick up your child by 5:45. •Parents and caregivers are responsible for supervising their children in the schoolyard once they have been signed out of their After School program.” • Children in the 4th/5th grade may leave the After School program unaccompanied at the end of After School day, but only if their parent, guardian, and/or caregiver has already provided written authorization to a provider/teacher. • Parents, guardians, and/or caregivers are not permitted to enter the PS6 building during dismissal, per PS6 and Department of Education rules. Initial Dismissal – 5:00pm to 5:10pm Outdoors in the Yard • Each program has a designated pick-up spot in the yard that is labeled with the name of the program. Parents, guardians, or caregivers must sign their child out of their designated program. Only individuals authorized to pick up students will be permitted to sign out students. Self Dismissal 4th and 5th Grade Students are allowed to self dismiss with prior authorization from their parent or gaurdian. Should students decide to self dismiss and remain in the School Yard to play around with friends, it is understood that the After school, the school, or the providers will not be liable for any injury that may occur as a result therein. Extended Dismissal – 5:10pm to 5:45pm Indoors (Cafeteria) • After 5:10pm, children are brought to the cafeteria to play board games or receive homework help. • After School Coordination staff meet parents, guardians, and/or caregivers at the cafeteria entrance for pickup during Extended Dismissal. • Parents, guardians, and/or caregivers must sign out their child on the sign-out sheet. Only individuals authorized to pick up students will be permitted to sign out students. Inclement Weather Dismissal – After School Staff Discretion, via Three Exits to the Yard •Initial Dismissal time: In severe weather conditions (not just rain), children remain inside during dismissal time and are dismissed via one of the following three exits to the yard: 1) the K room (102/104) that leads to the yard 2) The cafeteria entrance via the yard 3) Entrance that leads to the lobby (next to the alley). Providers will be informed of which area to dismiss from making these areas consistent throughout the semester. • The decision on dismissal each day will be the After Coordinators to make. At the gate right before dismissal Coordination will tell waiting parents if the dismissal will be indoors and which providers will dismiss from which exit. Parents, guardians, or caregivers must sign out their child at the designated exit on the sign-out sheet, making sure to write down the pick-up time and their relation to the child. During mildly rainy weather we will dismiss normally via the yard. The Extended Dismissal Procedure for pickups after 5:10pm remains the same during inclement weather. On some school days PS6 special events may alter the dismissal process. In these instances families will be notified in advance. PART C. Off Site Programming By permitting a student to depart from PS6 premises to participate in an off-site activity, PS6 and its afterschool program do not assume responsibility or liability for activities, events, injuries, losses, or damages occurring after the student leaves School property. Parents/guardians acknowledge that participation in off-site activities is at their own risk and that the PS 6 Afterschool’s supervision and responsibility cease upon the student’s departure from School grounds, except as otherwise required by applicable law. The fact that a student leaves personal belongings, including bags or other property, on School premises while participating in an off-site activity does not create any duty of supervision or liability on the part of the School for the student’s activities off-site or for any loss of or damage to such property. Part D: After school Medical Policies, Nut-Aware Snack and Epipen Policy Students who are absent from the school day cannot attend after school that same day. All programs are nut-aware, and we encourage families to provide snacks that are nut-free for the safety of all students attending After School. Students that were absent from the school day cannot attend after school that same day. After School Providers cannot administer medications unless there is an emergency. After School providers, teachers, and Coordination staff are not permitted to administer medications to students, with the exception of Epipens in a medical emergency. Children who require an Epipen must bring their own and may only keep it onsite if their parent or guardian has provided a signed Medical Administration Form (MAF). Staff are able to perform general first aid. including bandages and ice packs. For all other medical needs, students will be brought to the school nurse. Our school Nurse, Sarah Krauss, is available until 4:00pm daily and can be contacted at SKrauss2@schools.nyc.gov I have read and agree to the Dismissal Procedures in Part B above. I have read and agree to the After School Medical Policy, Nut Aware Snack and Epipen policy in Part D above I understand that After School Coordination staff, program providers and teachers, and PS6 administrators may assist in supervising, redirecting, and supporting my child in response to behavioral or medical concerns that arise during the Afterschool Program.
